Q: What type of payment does ACS accept?

• Private insurance
• Certain HMO plans, only on a referral basis
• Medicare and Medicare assignment

To prevent duplication of services we request copies of medical records
from previous physicians, including cardiologists. Patients will be
asked to complete a medical history and registration form. Our office
will need original insurance cards to allow our staff to make a
photocopy. We request that patients notify our office 24 hours in
advance if they are unable to keep the scheduled appointment.
For follow-up visits, we advise that you make your appointment before
you leave the office. This will ensure that your appointment is
scheduled within the time period recommended by your physician.
